Abstract
Drug-drug interactions (DDIs) pose significant risks to patient safety, especially in populations with polypharmacy such as the elderly and those with chronic conditions. This article explores the clinical consequences of DDIs, the mechanisms behind these interactions, and strategies for their identification and mitigation. Through a comprehensive literature review, the article underscores the importance of robust pharmacovigilance, personalized medicine, and clinician education to reduce adverse drug events and improve therapeutic outcomes.
